Steps to Admission




  • Fax a completed enrollment application to 602-404-9900 or email enrollment@hope4teens.org. (Once application is reviewed and accepted, you will be notified)

  • Commit to an admission date

  • Gather needed documents and belongings. (See Personal Needs Checklist within application)


Decide on travel plans. Here are some choices:

1. Student flies into Phoenix Sky Harbor airport after confirming pick-up availability from our staff

2. Family flies or drives student to program

3. Arrange for our teen transport service to escort student to program (Additional cost)

Enrollment can take as little as 24 hours, year-round. We work closely with you to plan the transition into and out of the program. This ensures that your child's best interest is served academically, socially, and spiritually.
